Unemployment and premium: why act on basic insurance

Losing a job neither suspends nor reduces the insurance obligation: in Switzerland, every resident must keep LAMal cover. The premium does not depend on your income but on the insurer, the canton, the region and the chosen model. A drop in income therefore does not automatically ease the bill, which leads many insured persons to review their contract after a job loss.

Two levers exist to lower the monthly burden without touching the benefit catalogue, which is identical across all insurers in basic insurance. The first is to compare insurers and switch to a more favourable premium. The second concerns the deductible and the insurance model. Considered together with the cantonal subsidy, they often produce a significant saving in percentage terms.

Switching health insurer after a job loss

Basic insurance premiums vary widely from one insurer to another, even though benefits are set by law and identical everywhere. Switching insurer is therefore one of the most direct ways to lower your premium. All insurers are bound by the admission obligation in basic insurance: none may refuse your application or impose a reservation on grounds of health or employment situation.

The ordinary deadline is 30 November, taking effect on 1 January, with one month's notice. The cancellation must reach your current insurer before that date. Only cancel once admission by the new insurer is confirmed, in order to avoid any break in cover, which the law prohibits.

The extraordinary right of cancellation on a premium increase

If your insurer announces a premium increase, you have an extraordinary right of cancellation of basic insurance. You may then leave at the end of the month preceding the entry into force of the new premium, observing the one-month notice. This window is valuable when unemployed: it lets you react outside the annual deadline if your premium rises.

Adjusting your deductible when the budget tightens

The annual deductible of basic insurance ranges, for adults, between 300 and 2500 CHF. A high deductible lowers the monthly premium but increases the share you pay yourself when you need care. Conversely, a low deductible raises the premium while reducing the financial risk in case of a health problem. When unemployed, this choice deserves careful thought according to your real needs.

Beyond the deductible, you pay a retention fee of 10% on costs, capped at 700 CHF per year for an adult. If you rarely see a doctor, a higher deductible can ease the monthly premium but exposes you to larger costs in case of the unexpected. If care is foreseeable, a low deductible better protects the budget despite a higher premium.

The cantonal subsidy: a decisive factor

The individual premium reduction, known as the subsidy, is paid by your canton of residence to people with modest income and assets. A job loss that lowers income may open or widen this entitlement. The subsidy does not change your contract but reduces the premium you actually bear, sometimes substantially depending on your situation.

The conditions, amounts and procedures depend on each canton and are reassessed according to your new financial situation. It is therefore advisable to report your change of circumstances to the competent cantonal body as soon as you lose your job. The subsidy combines with a switch of insurer or deductible: together, these levers maximise the easing of your premium in percentage terms.

Alternative models and the concrete steps to follow

Alternative models (family doctor, telemedicine, care network) offer lower premiums than the standard free-choice model, in exchange for a guided care pathway. For a healthy unemployed insured person, they are often an attractive saving lever in percentage terms, without reducing the statutory benefits of basic insurance, which remain guaranteed.

In practice: first compare the offers, check your subsidy eligibility with the canton, choose a suitable deductible and model, then cancel within the deadlines. Observe the 30 November deadline for effect on 1 January, or use the extraordinary right in case of an increase. Always keep proof of dispatch of your cancellation.

Frequently asked questions

Does unemployment automatically reduce my health insurance premium?

No. The LAMal premium does not depend on your income but on the insurer, the canton, the region and the chosen model. However, a drop in income may open entitlement to the cantonal subsidy, which reduces the premium you actually bear. Comparing insurers and adjusting the deductible are the other levers available to you.

Can I switch health insurer during my period of unemployment?

Yes. All insurers are bound by the admission obligation in basic insurance: none may refuse you or invoke your employment situation. Observe the ordinary deadline of 30 November with one month's notice, taking effect on 1 January. Only cancel after the new insurer confirms admission, to avoid any break in cover.

How does the cantonal subsidy influence my choice of insurer?

The subsidy lowers the premium you actually pay, but it applies regardless of the insurer chosen. It is therefore well worth combining it with a cheaper insurer and a suitable deductible. Report your change of circumstances to the competent cantonal body as soon as you lose your job to have your entitlement reassessed.
